What to send for the Work Capability Assessment, how to ask for it, and a requested-received-sent log for the whole bundle - so the evidence that wins claims never gets lost in the post.
The Work Capability Assessment is decided on function, not diagnosis - so the winning bundle is not the thickest one, it is the one where every page shows what your conditions stop you doing. Most claimants either send too little, send the wrong kind, or lose track of what was requested, what arrived and what actually went in the envelope. This tracker fixes all three.
What is inside
The three rulesFunction, recent, relevant - what makes a page of evidence count for the WCA, and the quality-over-quantity principle: a focused bundle beats a 50-page archive.
How to ask, item by itemGP letter, specialist letters, fit notes, prescription list, care plans and more - who to ask, what it costs, and the one sentence that gets letters written in the words WCA decisions use, including substantial risk.
The tracker tableOne row per item with requested, received, copy-kept and sent columns - the whole bundle visible at a glance, nothing forgotten half-way.
Sending log + cover noteEvery envelope and upload logged with dates and tracking refs, plus a cover note block so the DWP mail room can match your papers to your file.

Between requesting a GP letter, waiting for clinic letters, photocopying and posting, a WCA evidence bundle has a dozen points of failure - and lost papers usually surface months later, at the worst possible moment. The tracker keeps the state of every item visible (requested, received, copy kept, sent), and the sending log records how each envelope went and under what tracking reference. Copies only, never originals; name and National Insurance number on every page; proof of posting for anything that matters.

What evidence do I need for an ESA or UC health claim?+
Evidence that shows function, not just diagnosis: a GP or specialist letter describing what your conditions stop you doing, recent fit notes, a current prescription list, care plans or mental health team reports where you have them, a symptom diary, and a signed statement from someone who helps you. Recent evidence carries the most weight, and a focused bundle beats a thick folder.
Should I send original documents to the DWP?+
No - send copies and keep the originals. Write your full name and National Insurance number on every page so nothing gets separated from your file, keep a complete copy of the bundle at home, and log everything you send. For anything important, use tracked postage or get free proof of posting at the counter.
Can I send evidence after posting my WCA50 form?+
Yes. Supporting evidence can be sent at any stage: after the ESA50 or UC50 has gone in, to the assessment provider before your assessment, and to the DWP or tribunal at mandatory reconsideration or appeal. Always include your name and National Insurance number so it reaches your file.
